We are now accepting new patients! Rebekah Peters, Nurse Practitioner has joined the Mount Forest Family Health Team. If you do not have a primary care provider close to home, please see reception to complete a Patient History Form or download the form below. You will then be contacted to set up an appointment! Priority will be given to those who live/work in the Mount Forest area.

Land Acknowledgement
The Mount Forest Family Health Team and Claire Stewart Medical Clinic are located on the traditional territories of the Anishinabewaki, which include the lands of the Odawa, Petun and Mississauga peoples. Specifically, we are near the home of the Mississaugas of the Credit First Nation.
We acknowledge this with a commitment to move beyond words. Grounded in our relationship of working to promote wellness here, our plan to nurture the whole community — people and environment — and our action of thoughtful reflection on our impact on the land and the river, this acknowledgement becomes a living practice.
